To collect information relevant to your dietetic consultation, this website may use secure online forms hosted by Jotform Inc. (www.jotform.com). These forms may include personal and health-related information such as medical history, dietary patterns, and consultation goals.

Jotform acts as a data processor on my behalf and stores form submissions on secure servers. All data transmitted through Jotform is encrypted and handled in compliance with UK GDPR and EU data protection regulations.

Information submitted via these forms is accessible only to me (as the data controller) and is used solely for the purpose of providing dietetic care and managing appointments. Data will not be shared with any third party without your explicit consent, except where required by law.
